Staff only reported 11 of 30 concerns to the Head over a period of thirteen years. The consequence of this failure - he was banned from teaching. Clearly the sanction didn't work. The Lampard Review into abuses at Stoke Mandeville turned out to be a pulled punch. Here is one of many similar allegations, a nurse ignored the claim of a patient in paediatrics . This is far from uncommon behaviour among staff responsible for children in Regulated Activities.

The DfE never evidences its claim that MR doesn't work but it avoids discussing MR in Regulated Activities.
