The Spinoso Podcast
In 2017, I was living in 150 square foot bedroom in a house that was rented out to five other people and I was serving as a doctor to inmates in Nevada. Today, I live in suburban, Dallas and run multiple 7 and 8 figure businesses. I'm here to tell you – what I have done, you can do. The purpose of my podcast is to motivate you to take ownership of your life, to show you how to operate on elite levels in business and life, and to embrace the mindset that you can overcome anything that stands in your way. It's time to go after what you want - and get it.

Life Happens For You, Not To You (And I Can Prove It) - EP327 30.10.2026 7:09
Life happens for you, not to you, is the one belief Dr. Alex Spinoso would keep if he lost every other framework. Here's why he means it in his bones. If he had to throw out every book, quote and framework he's ever taught and keep a single idea to carry through the rest of his life, this is the one. But this isn't bumper-sticker positivity. In this episode of Medical Empires, Dr. Alex Spinoso ear...
How to Stop Your Clinic From Running Your Life - EP326 28.10.2026 42:41
You started your practice for freedom, more money, your own schedule, a higher standard of care. Then somewhere along the way it flipped and now you're the provider, the biller, the HR department and the janitor, doing notes until 10pm and starting it all over the next day. In this keynote to a room of nurse practitioners and clinic owners, I break down how to take your life back and build a pract...
Dolly Parton Said No to Elvis: The Business Lesson That Made Her Rich - EP325 23.10.2026 7:33
Everyone remembers the wigs, the rhinestones, the big laugh, the "dumb blonde from the mountains." Almost nobody realizes that was the strategy. Dolly Parton pulled off what I'd argue is the single greatest magic trick in business and in this one I break down how she did it and why it quietly made her one of the wealthiest self-made women alive. The centerpiece is a story that should be taught in...
Why I Walked Away From a Nearly $500M Offer for My Medical Business - EP324 21.10.2026 1:46:35
We had a 10x offer on the table for Genesis Lifestyle Medicine, almost half a billion dollars and we walked away from it. I sat down with Josh Ponton on ASHER Health's Standing in the Gap and told the full story: why the math didn't make sense, the multi million dollar stupid tax I paid learning how to sell a company and the month Brent and I spent wallowing in self pity asking ourselves if we eve...
The $360,000 Decision That Made Sylvester Stallone - EP 323 16.10.2026 6:30
In 1975 there was a broke, 29 year old actor with about a hundred dollars to his name, no car and a face partially paralyzed from birth that gave him a slurred way of talking every casting director in Hollywood used as a reason to say no. He was so broke he had to stand outside a liquor store and sell his own dog because he couldn't afford to feed it. That man was Sylvester Stallone and what he di...
The Smartest Way to Grow Your Med Spa (Most Owners Get This Wrong) - EP 322 14.10.2026 57:39
I joined Neal Karski on the Aesthetics Millionaires Show for a conversation every med spa and clinic owner needs to hear, especially the ones who think the path to wealth is opening more and more locations. Because that's the exact mistake that nearly cost me everything. Most owners believe scaling means expansion: more clinics, more states, more rooms. I did too. I grew fast and found myself runn...
Genghis Khan Killed So Many People It Cooled the Planet - EP 321 09.10.2026 5:46
Here's a fact that sounds made up but isn't: the Mongol Empire killed on such a massive scale that scientists say it measurably cooled the entire planet. I open this one with that because it's the strangest doorway I've ever found into a conversation about anxiety, gratitude and perspective. Almost everyone I talk to is quietly drowning in it right now. AI taking their job, the economy, the politi...
How I Built a Medical Empire From One Rented Room - EP 320 07.10.2026 34:18
I sat down with Chris Kille on the Building Great Companies podcast and told him the whole story, including the part most people skip. Before Genesis Lifestyle Medicine was a nine figure company, my wife and I rented out our nice Palm Springs house with the pool and the mountain view and moved into a single rented room in a five bedroom house we shared with strangers so we could save every dollar...
Why Your Masterpiece Is Hiding in the Boring Work - EP 319 02.10.2026 6:22
There's a church in Barcelona called the Sagrada Familia and people walk in and literally weep. It doesn't look like it was built by human hands, it looks like it grew out of the ground. And the lesson behind how it was built is the most powerful thing I can teach you about discipline, consistency and why the boring work actually matters. That jaw-dropping, once-in-a-civilization masterpiece was b...
How to Survive the Early Years of Building a Business | Operator Standard Q&A - EP 318 30.09.2026 2:16:03
I got on an open forum call with a room full of operators and told them to ask me anything, no script, no filter. The very first question hit the thing almost nobody talks about: how do you survive the early years of building a business without burning out or losing your family in the process. Seven years in, with four young kids at home, I've lived every version of that grind, so we went straight...
Why Your Life Changes Without You Ever Noticing - EP 317 28.09.2026 5:59
If you've ever felt like your life, your business or your marriage slowly drifted and you never saw it coming, this is about self-discipline, consistency and why the small daily habits matter more than any big decision you'll ever make. Picture a long hallway. You walk in one end and the walls are bright, clean white. But the whole hallway is painted as a gradient, every foot one shade darker, so...
Niche Down or Stay Broke: The Truth About Focus | Operator Standard Q&A - EP 316 23.09.2026 1:46:38
I got on an open forum call with a room full of operators, contractors, tradesmen, a realtor and told them to ask me anything, no script, no filter. The first guy started explaining his business and I had to stop him because after two minutes I still had no idea what his company actually did. That's the problem quietly killing more small businesses than any recession. The first operator runs a rem...
Humanity's Greatest Loser Beat Poseidon (Here's How) - EP 315 18.09.2026 8:18
In Record of Ragnarok on Netflix, the gods put humanity on trial, greatest humans ever versus Zeus, Poseidon and the rest, and the fight I can't stop thinking about is Poseidon against a swordsman named Sasaki Kojiro. Not the strongest human who ever lived. Not the greatest swordsman. History's greatest loser. Kojiro lost every fight his whole life. And even after he died he kept replaying those l...
The Truth About Peptides, TRT and the Ozempic Boom - EP 314 16.09.2026 1:04:34
I sat down with Tony Whatley on the 365 Driven Podcast for a long, wide-open conversation and we ended up covering everything from my early years as a physician working in the California prison system, to walking away from the golden handcuffs to build a med spa company, all the way to the stuff everyone's whispering about right now: peptides, hormone therapy and the GLP-1 weight loss boom. If you...
How to Raise Kids With Strong Values While Building a Business - EP 313 11.09.2026 8:46
When I interviewed for my all boys high school, I mentioned in passing that my family ate dinner together every night. I didn't think it was special. That's just what we did. Nobody got up until everyone was finished. Afterward the priest pulled my dad aside and asked if it was really true, every night. My dad said yeah, every night, no matter what. The priest said something my dad never forgot: I...
The Hardest Truths About Scaling a Business | Operator Standard Q&A - EP312 09.09.2026 1:53:58
I got on an open forum call with a room full of operators and told them to ask me anything, business, hiring, scaling, partners, family, all of it. No script, no filter, just real questions from people actually building. This is the stuff that doesn't make it into a polished keynote. We covered a lot of hard truths in this one. The first one I hit them with up front: to get where you want to go yo...
I'm Old Enough to Buy Eggs (Stop Shrinking Yourself to Fit In) - EP 311 04.09.2026 7:42
Every Halloween growing up, my dad didn't just take us trick or treating. He dressed up too, in full costumes he hand sewed himself. And this grown man would walk right up to the door next to us and say trick or treat. People would look him up and down and go aren't you a little old for this? And every time he'd look them dead in the eye and say well, I'm also old enough to buy eggs now. Give me t...
How to Structure, Scale and Protect a Business From Scratch | Operator Standard Q&A - EP 310 02.09.2026 1:19:47
I sat down with a room full of operators from East Tennessee and told them to throw every business question they had at me. No script, no filter. Just real questions from people actually building and the same answers I'd give if we were sitting across the table. We covered a lot of ground in this one. When it actually makes sense to acquire another business versus pouring that money into what you...
Your ADHD Isn't a Disorder. It's a Ferrari Engine With No Track - EP 309 28.08.2026 7:03
When I was a kid I got diagnosed with ADD. Back then they called it something else but the story was the same. I couldn't sit still. I couldn't shut it off. My brain was going a hundred miles an hour in every direction at once, all day, every day. So the school did what schools do. They looked at my parents and said we think you should put him on medication, calm him down, get him to sit quietly i...
The Warm Body Trap That's Quietly Killing Your Clinic - EP 308 26.08.2026 20:29
Picture the Trojans, ten years into the war. Ten years of dead sons, food running low, hope running lower. Then one morning the Greek camp is gone, the ships are gone and standing at the gates is a giant wooden horse. One man, a priest named Laocoon, throws a spear at it and screams don't bring that thing inside our walls. They ignored him. They were too tired to be careful and that one bad night...
The One Broken Detail That Defines Your Whole Business - EP 307 14.08.2026 8:49
I was driving with a buddy a while back and we pulled up next to a Nissan Versa. Out of nowhere he goes, how much you want to bet the rear passenger hubcap is off? I said what are you talking about. He goes, just watch. We get closer and sure enough, rear passenger side hubcap, gone. Turns out his wife used to drive a Versa and that one hubcap would never stay on. So they started noticing them eve...
You Become What You Tolerate in Yourself - EP 306 07.08.2026 7:29
It's 11:14 at night. You're in bed, phone in your hand. You told yourself two hours ago you'd be asleep by 10:30. You're not. You scroll one more reel, then one more. You check email even though nothing's new. Nobody sees this moment but you. Your wife is asleep next to you. Your kids are down the hall. Your team is offline. The only person watching what you do right now in the dark is you. And th...
My Mom Has Dementia, What It's Teaching Me About Life - EP 305 31.07.2026 9:12
With dementia you don't lose someone all at once. You lose them a little bit at a time. Some days my mom knows exactly who I am. Some days I'm just a kind face sitting next to her. And that has forced me to sit with a kind of grief most people don't talk about, the grief that shows up while the person you love is still here. In this episode I get personal about what my mom's dementia has taught me...
The Membership Mistakes Costing Your Clinic Six Figures a Year - EP 304 28.07.2026 19:04
Can you say out loud right now exactly how much money is going to hit your account next month? Not a guess. Not "it depends on how busy we are." An actual number. Most clinic owners can't. And that one blind spot is quietly costing them six figures a year. I had a call in my medical mastermind with a clinic owner who had been open almost four years. Good clinician, steady patients, decent reviews,...
The New FDA Peptide Rules Every Med Spa and Clinic Owner Needs to Know 22.07.2026 48:41
The FDA just reclassified peptides and it is going to reshape how med spas and medical practices operate in 2026. In this episode I break down exactly what the new FDA 503A peptide rules mean for your med spa business, which compounds are still legal to prescribe and the four step framework I am using to protect my own clinic and actually grow from this instead of getting crushed by it. Most clini...
